2. Submit your refundable business investment of $150 – refunded when you reach $2,500 in sales within 90 days
   The $150 may be in the form of a check (made payable to Newton Manufacturing Company) or major credit card (MasterCard, VISA, American Express). This is refunded when you reach $2,500 in sales volume within 90 days from your sign-on date.
   
   
3. Complete & submit a W-9 Form
   Due to IRS policies, you need to download the W-9 Form, print and complete. Please mail your completed W-9 Form to:
